• A signed permission to release form. If the requesting school does not provide a release form then a Great Valley form can be used. This form is required to indicate your understanding and permission to release records.  A link for this form can be accessed below.

• Indication of which teacher(s) you would like complete the Great Valley teacher recommendation forms.

****Great Valley completes their own teacher recommendation forms based on most frequently requested information and acts as a standard way to assess our student's abilities. A letter is sent with the packet informing the school of Great Valley's use of their own form.

• A date that the forms must be received at the requesting school.( Completion of the forms and transcript preparation will take approximately 10 school days).
  •  Also indicate if you would like a copy of your child's transcripts sent to school.
